I have a simple perl script now available for download that (on a UNIX server) uses Samba to send a windows popup message to alert you of threshold crossing. It takes its configureation from a file, and you can define different formats of message. Sadly, the MRTG Threshold command can only be defined as a single script, but you can wrap it again in order to select different messages.

I'm looking into adding configurations so that you can specify which message format (and which recipient list) is used by regexps matching the Threshold Description.
